Health Records and Insurance

The Wellness Center requires all full-time students to carry health insurance, and to complete the enrollment/waiver process below for each academic year. All first-year students are required to complete a medical history/physical and provide documentation of all required immunizations.

Online Insurance Enrollment/Waiver Process

All Ursinus students registered for 12 or more credit hours are required to provide proof of health insurance OR purchase the student health plan option underwritten by United Healthcare. The insurance enrollment/waiver form will be available starting January 2, 2018. The deadline for completing the health insurance waiver or enrollment is January 27, 2018 at 4 p.m. EDT. PLEASE NOTE: You only need to complete the health insurance waiver/enrollment form if you are a new incoming student for Spring semester. 

You will need your UC username and password to access the Insurance Enrollment/Waiver form below. Make sure you are not logged into your student portal. Select the appropriate option and follow the instructions.

Insurance Enrollment/ Waiver Form   

All full-time students are required to carry health insurance. If a student is not otherwise insured, the College has arranged for basic coverage provided by United Healthcare.

The annual cost for the 2017-2018 year is $1557.00. The annual coverage is in effect from August 7, 2017 through August 6, 2018. The cost for Spring semester and Summer coverage is $929.00, effective January 1, 2018- August 6, 2018. Students will be billed for the plan and will be credited once proof of insurance is provided via the Insurance Enrollment/ Waiver Form link above.

Losing Your Health Coverage?

If you did not enroll during our open enrollment period and have since lost coverage under your original plan, you may qualify for a Qualifying Life Event. A qualifying life event is an event that triggers an open enrollment window for an individual to purchase the Student Health Plan outside of the scheduled open enrollment period upon providing proof of involuntary loss of coverage and payment, within 30 days of losing coverage. (example: marriage, divorce, loss of job, etc)

The enrollment form, check or money order and the letter of creditable coverage, must be received within 30 days of losing coverage. After 30 days, the student or dependents will no longer be eligible to enroll in the plan, until fall of the following school year.  Please click here for cost and enrollment information as a Qualifying Life Event.
