Health Records and Insurance

The Wellness Center requires all full-time students to carry health insurance, and to complete the enrollment/waiver process below for each academic year. All incoming first-year students and transfer students are required to complete a medical history/physical and provide documentation of all required immunizations.

Go to Confidential Health Record to access the required health forms that are due by January 22, 2019. PLEASE NOTE: Only new incoming first-year or transfer students for the Spring 2019 semester need to complete the health record forms by January 22, 2019. Students that entered in the Fall of 2018 do not need to complete these medical forms or the insurance process again for the Spring 2019 semester. 

Online Insurance Enrollment/Waiver Process

All Ursinus students registered for 12 or more credit hours are required to provide proof of health insurance OR purchase the student health plan option underwritten by United Healthcare. The insurance enrollment/waiver form will be available starting January 1, 2019 for new incoming first-year or transfer students starting at Ursinus for the Spring semester. Returning students do not need to complete this process until the summer again. New incoming first-year or transfer students, the deadline for completing the health insurance waiver or enrollment form is January 31st, 2019 at 4 p.m. EDT. Students will be automatically billed for the student health plan if the form is not submitted by January 31st, 2019. Your student account will be credited once proof of insurance is provided via the Insurance Enrollment/Waiver Form link below. 

Click on the Insurance Enrollment/Waiver Form link in red letters below. You will need your UC username and password to access the Insurance Enrollment/Waiver form below. Make sure you are not logged into your student portal. Do not use Microsoft Edge as your internet browser. Please use internet explorer, firefox or chrome for best results in accessing the form. If you still have technical difficulties after trying those browsers, call tech support at 610-409-3789. Select the appropriate option and follow the instructions.

Insurance Enrollment/ Waiver Form   

All full-time students are required to carry health insurance. If a student is not otherwise insured, the College has arranged for basic coverage provided by United Healthcare.

The annual cost for the 2018-2019 year is $1624.00. The annual coverage is in effect from August 7, 2018 through August 6, 2019. **NOTE** Any student entering as a full-time student in the Spring 2019 term can purchase the student health plan for Spring 2019 semester and Summer 2019 at a cost of $971.00, coverage effective January 1, 2019- August 6, 2019. Students will be billed for the plan and will be credited once proof of insurance is provided via the Insurance Enrollment/ Waiver Form link above.

Losing Your Health Coverage?

If you did not enroll during our open enrollment period and have since lost coverage under your original plan, you may qualify for a Qualifying Life Event. A qualifying life event is an event that triggers an open enrollment window for an individual to purchase the Student Health Plan outside of the scheduled open enrollment period upon providing proof of involuntary loss of coverage and payment, within 30 days of losing coverage. (example: marriage, divorce, loss of job, etc)

The enrollment form, check or money order and the letter of creditable coverage, must be received within 30 days of losing coverage. After 30 days, the student or dependents will no longer be eligible to enroll in the plan, until fall of the following school year.  Please click here for cost and enrollment information as a Qualifying Life Event.
